Activity Logs enable you to track your progress through the course of your participation in the Wellness Incentive Campaign. Print the Activity Log for a given week, and have it handy as you participate in your selected activity. You cannot write in the online version of Activity Logs. These documents require Adobe Acrobat version 3.0 for viewing and printing.

The Activity Logs are available for your personal use; you do not have to submit them in order to participate in the campaign.
